Effective Treatments for Wet Age-Related Macular Degeneration

Wet age-related macular degeneration (AMD) is a persistent eye condition that can lead to partial vision loss and visual distortion. It results from abnormal blood vessels leaking fluid into the central retina, known as the macula, which is essential for detailed vision. Although a cure is unavailable, early intervention with medical treatments can slow or halt progression. Some patients may even regain some lost sight with prompt treatment.

Pharmacological Options

Inhibitors that block angiogenesis are commonly prescribed to manage wet AMD. These drugs prevent the formation of new abnormal blood vessels by inhibiting specific growth factors.

Popular medications include brolucizumab (Beovu®), aflibercept (Eylea®), ranibizumab (Lucentis®), bevacizumab (Avastin®), and pegaptanib sodium (Macugen®), all approved by the FDA as vascular growth inhibitors. These are administered through injections directly into the eye, typically monthly, following numbing. Consistent treatment is crucial for effectiveness.

Photodynamic Therapy (PDT)

This therapy works well for specific forms of wet AMD by targeting abnormal blood vessel growth in the macula. During PDT, a drug injected into a vein reaches the eye, where it consolidates in leaky vessels. A laser then activates this medication, sealing abnormal vessels. Each session lasts about 20 minutes, and multiple sessions may be necessary depending on the case.

Laser Therapy

In select cases, laser photocoagulation can treat wet AMD. After numbing the eye, a specialized laser is used to coagulate leaking vessels. While it can prevent further deterioration, it might cause a permanent blind spot due to scarring, and abnormal vessels may re-emerge. Severe damage reduces the efficacy of this treatment.
