Essential Guidelines for Applying for a New Passport
If you're planning to obtain a new passport, there are specific procedures to follow. It’s recommended to apply in person using the DS-11 form in the following situations:
- First-time passport applicants
- Applicants under 16 years old
- If your previous passport was issued when you were under 16
- If your passport was stolen, lost, or damaged
- If your existing passport was issued over 15 years ago
For renewal applications not falling into these categories, you can submit a renewal online by completing the DS-8 form.

Steps and Requirements for New Passport Application

Download the DS-11 form from travel.state.gov. Carefully read the instructions and follow all directions when completing it. Once filled, wait for an acceptance agent to review your form before signing. Additionally, provide your social security number if applicable. If you don’t have one, submit a signed statement declaring, "I declare under penalty of perjury under U.S. laws that I have never been issued a Social Security Number."
You can opt for a larger 52-page passport free of charge by checking the box at the top of the DS-11 form labeled ‘52 pages.’
Proof of Citizenship - Your proof must be an original or a certified copy, along with a clear photocopy. Valid documents include:
- Valid, undamaged U.S. passport
- Birth certificate issued by the city, state, or county, showing your name, date, and place of birth, along with parent details and official seals
- Naturalization certificate
- Certificate of citizenship
Copy of Citizenship Documentation - Submit clear, single-sided photocopies of both sides on standard 8.5"x11" white paper. Ensure they are legible and in black and white.
Passport fees vary: approximately $120 for children and $165 for adults.
